Services Provided:
Lincoln Self Reliance, Inc. operates offices at three locations in Lincoln County: Diamondville/Kemmerer, Afton, and Thayne; and one location in Uinta County: Evanston. These three offices, with their respective residential homes, provide services to their respective counties and surrounding counties and areas. Click here for our 2020 brochure.
LSR provides services with funding from:
Wyoming Medicaid (adult and children) Developmental Disabilities Home and Community Based Waiver.
Vocational services with funding through the Division of Vocational Rehabilitation and Wyoming Workforce Services, School Districts, Private Pay, and Acquired Brain Injury Waiver.
Admissions and Eligibility:
Persons requesting services must have a funding source in place or seek eligibility for a funding source (i.e., Developmental Disabilities Waiver services, private pay, alternate funding sources.) For more info, see our Admissions & Eligibility page.
Day Habilitation (Community Integration):
LSR provides Day Habilitation (Day Hab) services at two locations in Lincoln County, in Afton and Diamondville. Adult individuals participating in this program receive assistance in acquisition and retention of self-help, socialization and adaptive skills. Day Habilitation focuses on assisting each individual to achieve maximum potential in the least restrictive environment. Skills are taught on site and in the community.
LSR provides Adult Day Services, Child Hab Services, and Community Integration services at three locations in Lincoln County and one location in Uinta County. At all day programing locations adults and children participating in the program receive assistance in acquisition and retention of self-help, socialization and adaptive skills.
Adult Day Services, Community Integration, and Child Hab Services through LSR provides the persons served with the following:
Community, Social, and Independent Living Activities
Vocational services and community based/supported employment
Transportation to employment, appointments, and events
Medication management
Training and guidance regarding daily living skills, meal preparation, budget/shopping, personal care/hygiene, and other areas
Other activities and services
Adult Day Services and Community Integration focuses on assisting each individual to achieve maximum potential in the least restrictive environment. Skills are taught on site and in the community. LSR encourages the participation of family, friends, and volunteers.
Residential Habilitation (Supported Living):
LSR provides Residential Habilitation services at two locations in Lincoln County, in Afton and Diamondville/Kemmerer. Adult individuals participating in this program must have their own place of residence. LSR staff are available on an as needed basis up to 24 hrs. a day to assist with medication management, meal planning/preparation, transportation services, employment services, etc.
(In-Home Support)
In-Home Supported Living provides opportunities for the individuals served to practice skill development in their home and community. Individuals participate in specific training to address areas of need such as functional deficits in self-help, daily living skills, mobility, learning, communication, self-sufficiency, survival, etc.
Residential Habilitation (Assisted Living Home):
LSR provides 24-hr. staffed homes where individuals with disabilities can reside. LSR currently operates homes in three locations in Lincoln County: Afton, Kemmerer, and Thayne. Services provided include transportation to employment and recreational events, medication management, budget training, meal preparation training, daily living skills training, etc.

(Rental Unit)
LSR offers a one bedroom apartment located at 130 Wright Street, Thayne, Wyoming for a individual desiring to receive Supported Living services while living on their own. The location is a newly finished basement apartment that includes a washer/dryer and full functioning kitchen. Utilities are included in the monthly rental fees. All customary furnishing that the individuals does not have access to may be provided by LSR.
Individuals living on their own in Supported Living do not require 24-hour supervision. but do require a range of community-based support to maintain their independence. Supports are provided to assist the individual with the acquisition, retention, or improvement in skills related to living successfully in the community. Based on the needs of the individual, services provided might include assisting with common daily living activities; performing routine household activities to maintain a clean and safe home; assistance with health issues, medications, and medical services; teaching the use of police, fire and emergency assistance; managing personal financial affairs; building and maintaining interpersonal relationships; participating in community life; and 24-hour emergency assistance.

In-Home Support:

Personal Care:
Personal Care services include assisting with eating, bathing, dressing, personal hygiene, safety and activities of daily living.
Respite Care:
Respite Care services are provided for adults at the LSR facilities in Afton and Diamondville. Respite services are available on a short-term basis for persons who are unable, unassisted, to care for himself or herself.
Vocational and Community Based Employment:
LSR is certified to provide vocational training to individuals with various disabilities. The focus of this program is to assist the person receiving services in preparing to obtain and retain employment in their community.

LSR employs persons served at both the Afton and Diamondville offices, providing paper shredding services to local businesses, such as banks, medical centers, etc. Diamondville also offers recycling services in their area and employs individuals served to provide this service. With the addition of a greenhouse in 2010 at the Afton DayHab, plans are to provide persons served with training on raising vegetables and produce to sell in the community and for their own consumption.
In addition to LSR's on-site vocational training and employment of persons served, individuals also receive assistance in obtaining/retaining employment and ongoing support for workers and volunteers, along with providing transportation to/from work and volunteer assignments.
